ADA Compliance & Privacy Statement

Due to confidentiality laws, the Office of Accessibility Services cannot provide initial contact prompting students to sign up for accommodations with our office. It is the student’s responsibility to self-disclose and sign for accommodations every semester. It is strongly recommended to do so at the start of each semester to ensure your accommodations are in place. Please note, accommodations do not automatically carry over from one semester to the next nor are they retroactive.

Step 1: Complete and Submit the OAS Student Intake Form

By filling out the OAS Student Intake Form, you give us formal notification that you are seeking reasonable accommodations due to a qualifying disability. The Intake Form allows us to get to know you a little better so we can assign you to one of our counselors and start thinking about what reasonable accommodations might be a good fit for you.

Step 2: Provide Documentation of a Qualifying Disability

Documentation gives us a “road map” to follow when determining reasonable accommodations. Documentation can be uploaded directly to our secure system, Accommodate, as part of the Intake Form, or provided separately via email or in-person drop off (we do not recommend mailing disability documentation).

The most common forms of documentation are:

A copy of your most recent IEP/504 plan, including the psycho-educational or neuropsychological evaluation if one was conducted (this can be obtained from your high school)
OR
Documentation from a medical or mental health professional. Appropriate documentation will:
  • Include a statement of diagnosis
  • Describe the current academic impact of the diagnosis
  • Be signed by the provider, or the provider’s supervisor as appropriate, including their specialty and license number

Step 3: Make an Appointment With Your Assigned Counselor

After our team has reviewed and accepted your documentation, you will receive a letter with the name of your assigned counselor. Once you receive that letter, please:

  • Log into your Accommodate Account using your SUNY WCC username and password.
  • Select “Appointments” from the left-hand menu and choose NEW APPOINTMENT
  • Type of Appointment “INTAKE”
  • Select “Date Range”
  • Select “Time Range”
  • Select Location “In-person or Virtual”
  • Once this is done available dates and times will appear on the right-hand side of the screen – choose what works for you
  • Select “Submit Request”
