Report an Incident
Westchester Community College values the health, safety, and well-being of its members, seeks to uphold the integrity of our academic pursuits, upholds the rights of others, and ensures community members are living up to their responsibilities. When an individual feels a member of the college community has acted in a manner that is contrary to these tenets, they are strongly encouraged to file a report so the college can initiate the appropriate response processes.
The college has several different types of reports it collects. However, if you are not sure which form to submit, please use the General Incident Report to share your information with the college.
It is important that you fill in as many of the fields on the form as you know. Please be as complete as possible as it will provide the College with the best opportunity to respond to this incident.
Each report will be seen by officials at Westchester Community College. In all reports, you may chose to withhold your identity and report anonymously; however, doing so may limit the college’s ability to respond.
These reports are reviewed during normal business hours and should not be used if you need emergency assistance. If you need IMMEDIATE ASSISTANCE FOR AN EMERGENCY SITUATION, please call the 24/7 Campus Security Emergency Line at 6-911 from any campus phone, or (914) 606-6911 from any cellphone or outside line.

|Academic Dishonesty Report||Faculty||This form documents the outcome of the process at the conclusion of the process detailed in the Academic Honesty Policy.|
|Classroom Misconduct Report||Faculty||This form documents the outcome of the process at the conclusion of the process detailed in the Classroom Conduct Policy.|
|General Incident Report||Students, Faculty, Staff, & Community Members||This form files a report for incidents that do not meet the other criteria on this webpage or when community members are unsure where to file a report. Reports made using this form will be forwarded to the appropriate individual/department for review.|
|Hate or Bias Related Incident Report||Students, Faculty, Staff, & Community Members||This form files a formal complaint about a faculty member, staff member, or student regarding alleged discrimination based on race, color, creed, national origin, sexual orientation, or disability.|
|Sexual Violence/Misconduct Incident Report||Students, Faculty, Staff, & Community Members||This form reports an incident or suspected incident of sexual violence/sexual misconduct that can include, but is not limited to acts of domestic violence, dating violence, sexual harassment, stalking or sexual assault as detailed in the College’s Sexual Violence Response Policy.|

|Student of Concern Referral (CARE Team Referral)||Students, Faculty, Staff, & Community Members||This form reports observable behaviors that cause one to be concerned about a student’s personal well-being or lead one to believe a student may be a threat to themselves or others.|
