Community Health & Case Management Certificate
The Community Health and Case Management Certificate prepares students to work on the frontline in the fields of public health, health care and human services. Coursework is focused on the theoretical knowledge, ethical practices and core competencies needed to assist individuals and families in need of medical, mental health and social services. Students will learn how to work with their clients in creating service plans, identifying resources, and developing strategies for the implementation of services. The certificate includes courses and fieldwork where students learn to apply their knowledge and skills in real work environments.

Transfer Pathways
Those who complete their certificate and decide to continue their studies may transfer their completed course credits into the Health Studies A.S., Human Services A.S., or Human Services A.A.S.
After successfully completing your studies at Westchester Community College you can transfer almost anywhere, in-state or out-of-state. NYS residents are also guaranteed admission to a SUNY four-year campus.
