The Faculty Student Association of
Westchester Community College (“FSA”) is pleased to inform you of an
improved Accident and Sickness insurance plan offered to all Full Time and
Part Time Students at a reasonably cost. The FSA has made significant
improvements to the insurance plan this year including the following just to
name a few: In March 2010 the Patient
Protection and Affordable Care Act (PPACA), the blue print for national
health care reform, was signed into law. Student Health Insurance programs
are required to comply with the act. Because of this, certain benefits must
be changed for the 2012-2013 school year. These changes will provide
enhanced coverage or services to students who participate in the plan.
Following are some of the more important changes
that insurance companies must implement for the 2012-2013 academic year:
- Insurance companies must cover all
essential benefits including hospitalizations, emergency medical care,
laboratory services, mental health care and prescription medications. A

- Preventative services such as:
vaccinations, physicals and screenings as well as contraceptives must be
covered and cannot include co-pays, deductibles or co-insurance

- Pre-existing conditions cannot be
excluded for any covered persons younger than the age of 19.
It is very important that all students refer

benefits, limits and exclusions.
POLICY TERM The insurance
under Westchester Community College's Student Accident and Sickness
Insurance Plan for the Annual Policy is effective at 12:01 a.m. on September
1, 2012. An Insured Student's coverage becomes effective on that date or on
the date that the application and full premium are received by the
Company or Plan Administrator, whichever is later. The Annual Policy
terminates at 12:01 a.m. on September 1, 2013 or at the end of the period
through which the premiums are paid. The Spring semester coverage is
effective at 12:01 a.m. on January 1, 2013 and terminates at 12:01 a.m. on
September 1, 2013 or at the end of the period through which premiums are
paid.
ELIGIBILITY All Full-Time and
Part-Time Students of Westchester Community College are
automatically covered for Basic Accident Medical Expense Benefits. All
Full-Time Students are covered 24 hours a day. All Part-Time Students are
covered while participating in activities required of them by the College on
or off campus. An Insurance fee is assessed at registration for this
mandatory coverage.
All full-time students of
Westchester Community College are eligible to enroll in the Voluntary
Student Sickness Insurance Plan as described in the outline of coverage.
This coverage is in effect 24 hours a day. If a Student wishes to purchase
these benefits, please complete the on line Enrollment Form located at
www.ajfusa.com/students no later than October 17, 2012 for annual
coverage at a rate of $956 and no later than February 15, 2013 for spring
coverage at a rate of $695.
All part-time students of
Westchester Community College with at least 6 credits are eligible to enroll
in the Voluntary Student Sickness Insurance Plan. If a part-time student
wishes to purchase these benefits please complete the on line Enrollment
Form located at
www.ajfusa.com/students no later than October 17, 2012 for annual
coverage at a rate of $1,156 and no later than February 15, 2013 for spring
coverage at a rate of $798.
DEPENDENT COVERAGE Please
refer to the brochure located at
www.ajfusa.com/students for complete details regarding the dependent
coverage rates.

- Select your college if you’re Full-Time
or Part-Time from the drop down box and then enroll in the coverage.
You may also enroll by completing the
enrollment form online, printing and returning the enrollment form with a
check or money order to The Allen J. Flood Companies, Inc. at 2 Madison
Avenue, Larchmont, NY 10538. *Please note any enrollments submitted after
the deadline will be covered from the date after the Plan Administrator
receives both the application and premium. Premiums will not be pro-rated.
