Withdrawal Form for Online classes only

This is not a course drop form. This is the withdrawal form. If you are dropping your course and are expecting a refund please visit this link for more information. No refunds will be issued when using this form.

Make sure you fill out all of the required fields marked with an asterisk*.

Please note that all on-campus and extension site withdrawals are done through the registrar’s office. Call 914-606-6810 for their hours.

Note: It is College Policy that any student who "drops out" of a class without officially withdrawing must be given a grade of F.

You will receive an email confirming your withdrawal request. If you do not receive an email confirming your request within 2 business days, please call the Online Education Office.

	I was able to use the computer and log in with no technical difficulties.
	I would have done better if this course were taught in the classroom.
