Immunization
Requirements
Students born
on or after January 1, 1957 who wish to register for more than 5 credits in
any single semester are required to show proof of immunization before
registering.
Immunization Requirements:
- Measles Immunization
- Two injections on or after January 1,
1968 (at least 30 days apart) administered on or after the student's
first birthday. Immunizations administered prior to January 1, 1968 are
acceptable if there is proof that a live vaccine was used.
- Or provide the date of the
physician-documented disease.
- Or provide a Positive Immune Titer.
- Mumps Immunization
- One injection on or after January 1,
1969 administered on or after the student's first birthday.
- Or provide the date of the
physician-documented disease.
- Or provide a Positive Immune Titer.
- Rubella Immunization
- One injection on or after January 1,
1969 administered on or after the student's first birthday.
- Or provide a Positive Immune Titer.
(Proof of disease not acceptable)
- Meningitis
The student*
must indicate one of the following on the Immunization Form:
- That the student has had the
meningococcal vaccination within the past ten years or...
- An acknowledgement of the meningococcal
risks and refusal of the vaccination.

parents/guardians of students if student is under the age of 18

A
Titer is a blood test that can be performed to verify
the past administration of certain immunizations. |
Proof of immunization must be submitted to
the Student Health Office in the Student Center prior to working with a
counselor and registering. If you have already submitted this information
during a previous semester and were cleared by the College Nurse at that
time, you do not have to submit your immunization record again.
