Registration-Related Forms

Credit Course
Registration Form (Spring & Fall)

Use this form when registering for any courses for which you intend to receive credit. Remember, unless you are registering for one of Westchester Community College's summer semesters the completed form must be delivered in person to the Registrar's
Office.

Non-Credit
Registration Form

This form should be used for all Non-Credit course registration.

Application for Certificate of Residence

If you are a New York State resident living in a county other than
Westchester, you must submit a Certificate of
Residence. New York State residents of counties other than Westchester must
obtain a Certificate of Residence from the Chief Fiscal Officer of their
home county and submit it at the time of registration or pay the
non-resident tuition rate. Click for more information on the Certificate of Residence.

Combined Immunization/Meningitis Form

Students born on or after January 1, 1957 who wish to register for more
than 5 credits in any single semester are required to show proof of
immunization before registering. Proof of immunizations for Westchester Community College is
required only once. Click for more information on required immunizations.

Change of Address
Form

Be sure to notify us if you have a change of address. Don't miss out on
important information such as grades and new course schedules.
