Academic dishonesty is both an academic matter between a student and their faculty member and a violation under the Student Conduct Code. The Academic Honesty Policy, governed by Academic Affairs and found in the College Catalog, details action that can be taken by a faculty member. Actions taken by a faculty member are considered academic in nature and not a college disciplinary sanction.
This form should be used at the conclusion of the process detailed in the Academic Honesty Policy. It should be completed by either the facutly member (if the student does not appeal) or the Dean of the School (if the student appeals the decison). This report should include all documentation related, reviewed or created.
This form will be used to formally record this incident and allow the college to identify any patterns of misconduct.
In addition, the faculty member or Dean submitting this form may request that action be taken via the Student Code of Conduct if they believe such action is warranted. At the sole discretion of the Director of Student Support Services or designee, the student can be referred through the college conduct process for possible college sanctions as outlined in the Student Code of Conduct.
